Care in Context: Advance Care Planning and the Role of Social Work

  • Our current healthcare system is fragmented and in need of transformation. Care is inequitably delivered and too often not congruent with patient preferences. Advance care planning conversations lay the foundation for person-centered, family-focused, culturally congruent, goal concordant quality care. Although ideally, occurring over a lifetime, advance care planning conversations are an essential element of primary palliative care and especially important for those who are seriously ill. Social workers have the clinical background to provide nuanced advance care planning conversations and are often well positioned to offer leadership in developing advance are planning programs within their organizations. This interactive workshop will explore best practices in advance care planning with a focus on the social work role. Advance care planning tools and resources will be provided.

    LEARNING OBJECTIVES

    Social workers who take this class will be able to:

    1. Identify how the social work role contributes to effective advance care planning (ACP)
    2. Understand how the provision of contextualized, goal-concordant care contributes to lessened moral distress for patients, families and our teams.
    3. Identify next steps that you might take to improve ACP within your area of influence.

    Shirley Otis-Green, MSW, MA, ACSW, LCSW, OSW-C, FNAP, FAOSW

    Shirley Otis-Green’s career is dedicated to transforming the delivery of care to more equitably address the symptoms and stress of serious illness. Her education, research and consultation efforts focus on quality-of-life, palliative care, and interprofessional leadership. As Principal Investigator on studies with over $3.5 million in external funding, her work has been disseminated through more than 100 publications and 500 professional presentations.

    Shirley is a National Association of Social Workers Pioneer and has been recognized with Fellowships by the Association of Oncology Social Work, California Health Care Foundation, and the National Academies of Practice. She was among the first to receive an international Master of Arts in Health Research - Palliative Care degree from Lancaster University in Great Britain, and is co-editor of The Oxford Textbook of Palliative Social Work, now in its Second Edition. Shirley is also the Founder and Consultant for Collaborative Caring.

    Completion of this class will result in the receipt of three (3) continuing education hours. CEHs are not awarded for partial completion of the class.
